245D Waivered Services
Abukar HHC is a 245D licensed provider of 24-Hour Emergency Assistance, Adult Companion, Homemaker, Respite, Night Supervision, Individual Community Living Support, and In-home Supports/In-home Family Supports including Individual Home Supports with/without Training. Our coordinators work with clients to select qualified caregivers for each service. These services are covered by federal and state funded waivers and various Managed care organizations (MCOs).
Our Services
24-Hour Emergency Assistance
If you need help in an emergency, call 9-1-1.
24-Hour Emergency Assistance for people on a Disability Waiver is on-call counseling and problem solving and/or immediate response for help at a waiver recipient's home due to a health or personal emergency.
Covered services include:
* Immediate response for assistance at a person's home
* On-call counseling and problem solving
* Development of self-advocacy skills
Adult Companion
Adult Companion Services are non-medical social support services for older adults who need supervision, care or assistance.
When a person is on a disability waiver or the Alternative Care (AC) program and over the age of 18, companion services could include:
* Laundry
* Light housekeeping tasks
* Meal preparation
* Shopping
* Supervision
* Socialization
Homemaker Services
Homemaker Services help with general household activities when a person is unable to manage the home, or when the person regularly responsible for these activities is temporarily absent or unable to manage the home.
All licensed Homemaker services may provide:
* Help with routine household tasks and cleaning
* Monitoring safety and well-being
For people on the waivers or Alternative Care Program, Activities of Daily Living (ADL) or Home Management may be provided under the Homemaker service when providers have the appropriate level of licensure.
* Home Management
- Arranging for transportation
- Meal preparation
- Shopping for food, clothing and household supplies
- Simple household repairs
- Laundry
* Assistance with Activities of Daily Living
- Help with bathing, eating, dressing, mobility, transferring from bed to chair and toileting
Respite Services
Respite provides care on a short-term basis for a person who cannot take care of themselves, and who is usually taken care of by a family member or friend. The service is designed to provide temporary coverage for the caregiver during an absence or need for relief.
In-Home Family Supports
In-Home Family Supports (IHFS) help people with disabilities live in the community more independently. It is available through the disability waivers - Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), and Developmental Disabilities (DD).
Training is provided in the person's home and community, with the goal to develop, maintain or improve the person's skills related to activities of daily living.
Training is available to the person on the waiver or the person's family, including extended family.
Night Supervision
Night Supervision is overnight assistance and monitoring in the home of a person for a period of no more than twelve (12) hours in a 24-hour period.
Night supervision carries out behavior programming and plans about living skills and interventions for the person and include the following:
* Carrying out the person's positive support plan
* Reinforcing independent living skills
* Help with incidental daily activities
* Help with instrumental activities of daily living
Individual Community Living Supports (ICLS)
Individual Community Living Supports (ICLS) offers assistance and support to Elderly Waiver (EW) and Alternative Care (AC) participants who need reminders, cuing, or intermittent or moderate physical assistance to remain in their own homes. A qualified worker will deliver a range of individualized supports.
Waiver and Alternative Care Programs
People with disabilities and older adults may qualify for waiver/AC programs if they choose to live in the community and require certain levels of care. The following programs are available to people who meet eligibility criteria:
Alternative Care (AC): For older adults who require the level of care provided in a nursing facility and who are not yet eligible for MA.
Brain Injury (BI) Waiver: For people with a traumatic, acquired or degenerative BI who require the level of care provided in a nursing facility that provides specialized services for people with BI, or who require the level of care provided in a neurobehavioral hospital.
Community Access for Disability Inclusion (CADI) Waiver: For people with disabilities who require the level of care provided in a nursing facility.
Community Alternative Care (CAC) Waiver: For people who are chronically ill or medically fragile and require the level of care provided in a hospital.
Developmental Disabilities (DD) Waiver: For people with developmental disabilities or related conditions who require the level of care provided in an intermediate care facility for persons with developmental disabilities (ICF/DD).
Elderly Waiver (EW): For older adults who require the level of care provided in a nursing facility.
